Support Coordinators

LHC is committed to delivering high-quality, culturally inclusive, and participant-centred support. We work collaboratively with Support Coordinators to ensure seamless service delivery, clear communication, and compliance with NDIS best practices.

We provide support to NDIS participants of all backgrounds, including those requiring:

We aim to onboard new participants within 48 hours of receiving a referral. Our intake team works efficiently to assess needs, develop care plans, and commence support as soon as possible.

Yes! We provide detailed progress reports and work closely with Support Coordinators to ensure plan reviews reflect participant goals and evolving needs.

LHC follows strict NDIS compliance frameworks, ensuring:

Yes! Simply submit a referral via our website or contact our intake team. We will review the participant’s needs and begin the onboarding process immediately.

We primarily operate in Southeast and East Melbourne we also service areas in Melbourne, select regional Victoria, Gold Coast, Brisbane, and select regional Queensland

For urgent cases, we prioritise same-day assessments and aim to provide immediate support where possible. We encourage Support Coordinators to highlight urgent needs when referring participants.
